Preferred method of contact
Please check one of the boxes below to indicate if you want to contact the student or would prefer to have the student contact you.
You will be contacted by the student via your preferred method of contact (above).

Your faculty liaison will meet with you after March 30th to review your role as a Preceptor. Your assigned student will begin attempting to contact you in March.  Please watch for unfamiliar emails going to your spam folder, or unfamiliar calls/texts on your phone.

What to expect next:

Your name will be given to the Student Placement Coordinator at PeaceHealth, who will give it to your manager to be approved as a preceptor.  The accepted names will be given to the LCC Clinical Coordinator, Aimee Kruger, who will work with LCC faculty to assign the students.  Aimee will reach out to all who have applied to let them know if they were accepted as an LCC preceptor or not. If you are not accepted as one of the preceptors for LCC, and you have questions about that, please contact your manager.  Thank you for being a potential preceptor - we couldn't do this without you!

Other Options
If you know a student you would like to work with, please include that name in this box. Faculty will make that placement, if at all possible.
