Accessing Care

What do I need to do if I am a new patient?

All new patients need to complete the new patient paperwork before they can be seen by a provider through telehealth, a phone visit, or in-person in the clinic. New patient forms can be found here. 

What if I need to cancel my appointment?

It is very important to us to provide high quality care in a timely and efficient manner to all eligible LCC students and staff. It is difficult for us to meet this goal when patients repeatedly fail to keep scheduled appointments without notifying us. We understand that occasionally, unforeseen circumstances occur that make it difficult to keep an appointment. In those instances we would very much appreciate, whenever possible, 24 hours notice of your need to cancel or reschedule an appointment. Failure to notify us in advance means that your appointment time cannot be used for another patient. As a result someone who needs care may not be seen. Please be courteous to other patients and cancel your appointment rather than simply missing it. We may ask you to reschedule your appointment if you are 10 minutes late.

Do I have to pay for services at the Health Clinic?

Most services and all visits with a provider are no cost. Sports physicals are $50. Lab tests, immunizations, medications, and some procedures administered in the clinic are low cost (these are free for patients with OHP) and your healthcare provider will discuss the cost with you before administering any of these services. 

Any services that have a cost will be charged to your L account. 

What if I don’t have insurance? 

Many Lane students qualify for free medical insurance through the Oregon Health Plan (OHP or Oregon Medicaid). 

We can help you sign up or see if you qualify; please call for an appointment.

What if I’m not eligible to use the Health Clinic?

If you're a Lane student taking only online classes, you may now opt in to use the Health Clinic! You'll need to use the Health Clinic Services Registration form to opt in to pay the student health fee of $60.

What if I am worried about my privacy?

The Health Clinic values your privacy and the services you receive in the Health Clinic are confidential. Here is a copy of the Health Clinic’s Notice of Privacy Practices.

If you are on your parent’s or spouse’s insurance plan, Oregon guarantees you the right to have protected information about your health care sent directly to you from your insurer instead of to your parent or spouse. Here is the Confidential Communications Request form to fill out to send to your insurance company. Here is information about the Confidential Communications Request process. The Health Clinic can also answer any questions you may have about the process.

Does the Health Clinic have policies I should know about?

Yes! The Health Clinic has a variety of policies about the services we offer, cancellations, no-shows, privacy, and more. You can read about all of our policies here.

What if I need a prescription refilled?

You must already be a patient at the LCC Health Clinic to request a prescription refill. Prescription Refill Requests will be processed within 2 business days of receipt of the request. If you need to request a refill of a prescription obtained from a pharmacy, please call the pharmacy to request the refill. The pharmacy will forward the request to us by fax.

If you received medication through Patient Assistance, please follow the instructions given to you at the time of your application. If you have misplaced that information or have questions regarding patient assistance medications, please call the clinic at (541) 463-5665 with the following information: Your name, state that it is a patient assistance medication, name of the medication, name of the manufacturer of the medication, the amount of the medication you are taking, and the name of the provider who prescribed it to you.

The LCC Health Clinic does not provide after-hours medical care. If you need a prescription refill, contact your pharmacy.
